Eligibility Criteria
Educational Qualification: A candidate should have passed BSC nursing with the integrated curriculum of certification in community health for CCHN in order to apply for the position of Community Health Officer in Uttar Pradesh.
Age Limit: The age of the applying individual should not be more than 35 years to fill the application for the desired post.

Probation Period
The nominated candidates will have to participate in a four months training program in Certificate in Community Health for Nurses (CCHN). Moreover, they have to submit a surety bond of Rs. 2.50 lakh ( on the stamp paper of Rs. 100/-) prior to joining the training to serve for a minimum of three years at Sub Health Centre level Health and Wellness Centres in Uttar Pradesh upon completing the Certificate Course in Community Health for Nurses training. After successful completion of CCHN training, the appointment will be completely on a contractual basis for a duration of the one-year, renewable will be based on the satisfactory performance and the approval from the Government of India.
